When you use the contact or assessment forms we collect the information you choose to give us: your name, email address, telephone number, preferred contact method, the areas you are interested in, a short description of what you would like support with, a preferred day or time and your consent to be contacted.
We do not ask for, and ask you not to submit, medical or mental-health history through these forms. If such information is needed later, it will be requested through a separate, secure process with your explicit consent.
Our web server records standard technical information such as IP address, browser type and pages visited. This is used only to keep the website secure and working correctly.
We do not sell your information and we do not use it for automated decision-making or profiling.
Form submissions are stored on our website and sent to us by email. Our website is hosted by Hostinger and email is delivered through our mail provider. These providers process data on our behalf under contract. We do not share your details with anyone else unless the law requires it.
Enquiries that do not lead to an assessment are deleted within 12 months. Client records are kept for the duration of the working relationship and for up to 6 years afterwards where needed to meet legal, insurance or accounting obligations.
Under UK data protection law you can ask us to access, correct, delete or restrict the use of your personal information, object to its use, or provide it to you in a portable form. Where we rely on consent you can withdraw it at any time. Write to contact@bmrejuvenate.com and we will respond within one month.
